How Do I Obtain My Medical Records?
The Health Information Services department at AAMC fulfills requests for information for
medical records for all inpatient, outpatient, physical therapy, wound care and neurology visits.
The release of information staff will be happy to assist you in obtaining your medical records
upon receipt of a valid authorization. You can mail, fax, or personally deliver your authorization
to release health information.
Please note: Legal identification such as a valid photo ID, driver’s license, etc. is required to
obtain your medical records.
Request Your Records by Mail
Print the Authorization to Release Protected Health Information Form found below and mail it
with a copy of legal identification, such as a driver’s license, to:

Request Your Records by Fax
Fax the Authorization to Release Protected Health Information Form to 864-366-3345 and
include a copy of legal identification, such as a driver’s license.
Request Your Records in Person
You are welcome to stop by our office at the address above. The staff can assist you with
requesting your records. Health Information Services’ office hours are Monday through Friday,
8:30 a.m. to 4:30 p.m. Questions? Our number is 864-366-3266. Please bring some form of
