| Position Description: | The Program Director for Lakelands Cares (LRHN) is responsible for the strategic, programmatic, financial, and management operations of Lakelands Cares. Works collaboratively with primary care and specialty physicians, local hospitals, health and human service organizations, and other network providers to establish and maintain access to necessary healthcare services for low-income uninsured residents of the Lakelands region. Leads program implementation, policy development, fund raising, and strategic planning under the direction of the LRHN Executive Director. The program director reports to the LRHN Executive Director. |

 | The above wage range reflects the minimum, midpoint and maximum hourly rate for this position. New employees are hired between the minimum and midpoint of this range. The initial wage offered will be based on qualifications and experience. |

 | Key Responsibilities
· Refine, articulate, and advance Lakelands Cares strategic direction, working closely with the Executive Director and the Lakelands Cares Committee to establish priorities, drive the growth of Lakelands Cares, and improve effectiveness and efficiency within Lakelands Cares network providers to serve patients.
· Manage the program’s human resources by: determining qualifications, competence and performance expectations; implementing and monitoring staffing plans; providing orientation, in-service training and continuing education of staff; and applying policies/procedures pertaining to human resource management.
· Continuously assess and improve Lakelands Care’s performance by designing operational processes, monitoring performance through data, analyzing data, implementation of sustainable performance improvement, and developing a sustainability model for the program.
· Manage program budget including processes to monitor operational and capital costs.
· Lead fundraising efforts of Lakelands Cares, which include: cultivation and solicitation of support from foundations, corporations, individual donors, and government.
· Be the chief advocate for the organization’s mission and services. The Program Director is expected to be an active spokesperson to increase community and provider awareness of Lakelands Cares.
· Lead the efforts of Lakelands Cares in exploring linkages, partnerships, and collaborations with external organizations and groups to promote a coordinated delivery system.
· Develop policies and procedures that guide and support provisions of the program.
· Provide constant and thoughtful leadership and provide a forum for creative and innovative ideas. |
